Services Offered
The Health Department offers a variety of social work services which include Care Coordination for Children, Pregnancy Care Management, Family Care Coordination, and Baby Love Plus Community Health.
Baby Love Plus Community Health Advocate (CHA)
Baby Love Plus Community Health Advocates provide health education information to the community, community outreach, networking and connecting to services. They go into the community in a variety of ways to bring the information about vital services to the community.

Care Coordination for Children Services
Care Coordination for Children (CC4C) is a voluntary program for infants and children from birth to age 5 with developmental and/or chronic health problems. The CC4C Program is replacing the Child Service Coordination (CSC) program. The new program aims to improve the health outcomes for the highest risk children in our community.
Care Coordination Services will help the parent to:
- Develop appropriate bonding with their infant
- Help parents to reinforce positive play
- Assess developmental milestones
- Reinforce the need for medical follow ups and immunizations
Your medical doctor may discuss Care Coordination with you and refer you for CC4C services. You may also refer yourself if you are interested in the services. When a referral is made, you will be contacted by a Care Coordination specialist from the health department. Your Care Coordinator will help you develop skills that will make parenting easier.

Family Care Coordination (FCC)
Halifax County Health Department and Northampton County Health Department share in the provision of Family Care Coordination (FCC) Services.
The Family Care Coordination Program is open to women with at least one child in their home and encourages Interconceptual health and healthy pregnancy spacing. The goal is to help women to space their pregnancies for optimum health for both her and her babies.

Pregnancy Care Management
The Pregnancy Care Management Program (PCM) is designed to help you have a healthy pregnancy and a healthy baby. It is a voluntary program providing support, information and assistance in a variety of areas, to pregnant women in Halifax County who are eligible for Medicaid.
PCM services will be provided by a Pregnancy Care Manager working at your local health department or other provider agency. A Pregnancy Care Manager will establish a plan of care and a schedule of contacts with you based on your individual situation. The Pregnancy Care Managers will be working closely with your doctor to make sure that you are able to keep your medical appointments and follow your doctor's medical advice. Services are offered from conception through the pregnancy and on two months post-partum. The goal is to help ensure the best possible birth outcomes for mother and baby.
To receive Pregnancy Care Management services your doctor will refer you or you may contact the Health Department regarding Pregnancy Care Management.
