This position provides case management for Medicaid children, age birth through 5, who meet certain risk criteria for enrollment in the Care Coordination for Children (CC4C) Program. The main goals of CC4C is to improve health outcomes and reduce Medicaid costs for enrolled children. The case manager works with families to identify and access community resources to meet their needs and encourages and supports families in the development of skills needed for effective problem solving, making decisions, and enhancing the capacity of families to meet the special needs of their children. Other duties include assessment, planning, counseling, evaluation and reviewing records of clients in this program.
Any combination of education and experience equivalent to a Bachelor’s degree in social work from an appropriately accredited institution; OR Bachelor’s degree in a human services field from an appropriately accredited institution and one year of directly related experience; OR Bachelor’s degree from an appropriately accredited institution and two years of directly related experience. Prefer applicants with a degree from an accredited social work program. Other requirements include possession of a driver’s license valid in the state of N.C.
